This session will begin with a brief overview of the updated 2021 ADA guidelines, focused on agents recommended for compelling indications of increased ASCVD risk, heart failure and CKD.  We will analyze pertinent trials surrounding SGLT‐2 Inhibitors and GLP1 receptor agonists, compare/ contrast agents in each class and discuss how to handle these medications from a transitions of care standpoint. This will include criteria for initiation and monitoring upon hospital discharge. The presentation will conclude with a review of the phase II trial of once weekly insulin and a discussion about transitions of care aspects surrounding this new medication.

Learning Objectives for Pharmacists & Nurses  |  At the end of this session, participants should be able to:

1. Discuss indications and place in therapy for the new antihyperglycemic agents.
2. Identify key differences between the GLP‐1 analogs and the SGLT‐2 Inhibitors.
3. Describe the role of the new diabetes agents within a transitions of care plan.

Learning Objectives for Pharmacy Technicians  |  At the end of this session, participants should be able to:

1. Identify medications within the same class to ensure there is not duplication of therapies while obtaining medication history.
2. Outline common lab parameters used to assess the effectiveness of diabetes treatment.
3. Describe different storage requirements for recommended agents.
