Four-factor prothrombin complex concentrate (4F-PCC) has been utilized for various off-label uses, including direct oral anticoagulant reversal in patients experiencing life-threatening bleeding, perioperative management and coagulopathy reversal in patients with liver disease. Best practices are uncertain for off label uses of 4F-PCC due to limited data on the comparative effectiveness of reversal strategies and variability in patient-specific factors. Additionally, there have been controversies associated with dosing, timing, and appropriateness of administration. This presentation will provide education on off-label uses of 4F-PCC and analysis of available literature so that pharmacists can make informed clinical decisions and recognize the appropriateness of 4F-PCC use.

  1. Recall the pharmacology of four-factor prothrombin complex concentrate (4F-PCC) and its place in current guidelines.
  2. Identify recent studies that assess the safety and efficacy of 4F-PCC off-label uses.
  3. Recognize the role of the pharmacist in evaluating the appropriateness of 4F-PCC use.
