Clinical trials have proven the superiority of generic dabigatran in comparison to warfarin in significantly reducing ischemic and hemorrhagic strokes. Other studies comparing the efficacy of dabigatran to other direct oral anticoagulants (DOACs) (i.e., apixaban and rivaroxaban) have suggested equivalency as well. In clinical practice, the use of DOACs have increased as they have been statistically and clinically proven effective in preventing and treating thromboembolic events. While these agents are beneficial in several cardiovascular disease states, some are associated with a greater cost burden than others. Although apixaban and rivaroxaban are more commonly prescribed, recent studies comparing these agents to dabigatran have shown similar effectiveness in preventing thromboembolic events. In this session, the clinical indications for each will be outlined and cases in which substitution is appropriate will be assessed.

  1. Recall current evidence-based guidelines that assess the clinical indications and effectiveness for dabigatran, apixaban and rivaroxaban
  2. Identify financial consideration in the use of dabigatran, apixaban, and rivaroxaban
  3. Recognize cases in which generic dabigatran may be an appropriate substitution for apixaban and rivaroxaban
