Severe brain injury due to trauma is associated with high mortality. Approximately 50,000 people die from a traumatic brain injury (TBI) each year in the United States. Mortality from TBI is usually due to several factors including, elevated intracranial pressure (ICP), severe intracranial hemorrhage and coagulopathy, impaired oxygenation and ventilation, and seizures. Management of TBI is directed towards each of these factors with ICP being a central focus. Use of hyperosmolar agents, such as mannitol and hypertonic saline, is the main treatment modality in reducing intracranial pressure. The optimal agent of choice is controversial based on current data and unique characteristics of each agent. The intent of this educational session is to educate clinicians on current recommendations on managing TBI and to inform providers of the current clinical data surrounding the use of hyperosmolar agents. Throughout this activity, the presenter will provide clinicians with scenarios for potential use and identify notable information when evaluating a patient with TBI.

Learning Objectives for Pharmacists and Nurses  |  At the end of the session, participants should be able to:

  1. Describe the literature evaluating the use of hyperosmolar agents in the treatment of traumatic brain injury and identify areas for future research.
  2. Identify patients with traumatic brain injury for which treatment with hyperosmolar agents would be appropriate.
  3. List the potential risks and benefits of using hyperosmolar agents in the treatment of traumatic brain injury.
