HealthTrust & Abbott present…”Improving Patient Outcomes & Decreasing Hospital Costs Through Nutrition”

Up to 50 percent of patients are malnourished upon admission to an acute care setting. This can impact clinical and financial performance measures throughout the patient journey, including lengths of stay, costs of care, mortality rates and readmissions. In this program,  KentuckyOne Health and CHI Memorial Health will share how their healthcare data assets were leveraged to implement a real-world Nutrition Quality Improvement Initiative (NQII) that included rapid identification of and intervention with malnourished patients by way of an electronic medical record, caregiver nutrition education and post-discharge nutrition care. With this initiative, the median time from patient hospital arrival, admission and Malnutrition Screening Tool (MST) to Oral Nutrition Supplements (ONS) initiation was reduced. There were also decreases in lengths of stay and readmission rates. After the NQII’s implementation, identification and diagnosis of patients with malnutrition increased from 3.14 to 6.84 percent (p<0.01).
