Join us for a discussion of how local health departments (LHDs) are working with community stakeholders to build partnerships around the social determinants of health (SDOH) and Healthy People 2030. The United States Department of Health and Human Services (HHS) Office of Disease Prevention and Health Promotion (ODPHP) and the National Association of County and City Health Officials (NACCHO) will facilitate a conversation with LHD and community-based organizations (CBOs) that will provide attendees examples of how communities have come together to partner on work related to addressing the SDOH utilizing the Healthy People 2030 framework.

This webinar will provide a brief overview of how Healthy People 2030 addresses the SDOH, specifically how it’s beneficial to moving work upstream, demonstrate how select Healthy People 2030 objectives are aligned with SDOH, and provide a platform for LHD and CBO partner pairs to discuss their partnerships and the work that they have been able to accomplish to address the SDOH using the Healthy People 2030 framework.

  • RADM Paul Reed, M.D., Director, Office of Disease Prevention and Health Promotion, Deputy Assistant Secretary for Health, United States Department of Health and Human Services;
  • Peter L. Holtgrave, Senior Director, Public Health Infrastructure and Systems, National Association of County and City Health Officials; and
  • Local health departments and community-based organizations who have utilized the Healthy People 2030 framework and SDOH to guide their work.

Healthy People 2030 sets data-driven national objectives to improve health and well-being over the next decade. The initiative also includes other components that help track progress toward achieving the Healthy People vision. Healthy People 2030 includes a wide range of objectives developed by workgroups made up of subject matter experts in specific topics. Most Healthy People objectives measure progress towards a target over time, but some aren’t measurable or have other limitations. To learn about how objectives are related to Leading Health Indicators, Overall Health and Well-Being Measures, and the Healthy People 2030 vision, check out our Healthy People 2030 Objectives and Measures graphic.
