“The Development and Success of a Transitions of Care Program at a Community Hospital”

Hospital readmissions have a significant impact on the healthcare system. The Centers of Medicare and Medicaid Services (CMS) continue to expand their criteria of disease states that will be penalized for readmissions. Transitional care involves three domains of transfer: information, education and destination. Medicare quotes a 19 percent all-cause 30-day readmission rate, of which 47 percent may be preventable. Additionally, approximately 65 percent of adverse drug events (ADEs) that occur post hospital discharge are medication related. In current literature, pharmacist involvement has been shown to decrease ADEs and increase patient satisfaction. In this session, attendees will learn a transitions of care program to achieve goals to helpdecrease readmission rates.

Learning Objectives:

  1. List the goals outlined by the Centers of Medicare and Medicaid Services and The Joint Commission for transitions of care (TOC).
  2. Describe the role of pharmacists in the advancement and implementation of a TOC program.
  3. List and describe the metrics used for a TOC program, including medication related interventions, pharmacy services time, recommendation acceptance rates by other providers, and overall rates of readmission.
