Effective medication reconciliation is crucial for safe medication use. Pharmacy personnel can play an important role in accurately documenting home medications, reconciling medication orders, providing additional recommendations on appropriateness of therapy, and decreasing the burden on nursing staff. In 2023, one 159-bed hospital in Tennessee initiated a successful pharmacy-driven medication reconciliation effort, swiftly meeting enterprise standards and enhancing patient care with interdisciplinary collaboration. Also in 2023, other facilities in the Far West Division (FWD) with established Pharmacy-Led Medication Reconciliation programs created the FWD Med Rec Quality Improvement Workgroup to facilitate a collaborative approach to identifying and implementing quality improvement initiatives. Presenters will share tools that attendees can utilize to assess and enhance their facility’s med rec process—whether establishing a program from the ground up or improving an existing one.

  1. Recall key considerations in developing a tailored Pharmacy-led medication reconciliation program and review process for quality improvement initiatives  to enhance patient safety.
  2. Identify the potential structure and operations required to establish a collaborative Medication Reconciliation Quality Improvement Workgroup.
  3. Recognize strategies for implementing medication reconciliation quality improvement initiatives within a facility or health system.

  • Members who attended the live HTU event and applied for CE credit are not eligible to receive credit for the same program in a virtual format.
  • If you plan to obtain credit for a live session, stay until the end of the presentation to receive the verification code you will need to obtain the continuing education credit. CE credit is individual. No group credit is available. Each person must register for a webinar and attend the webinar in its entirety in order to receive an email after the event with instructions on how to apply for continuing education credit. Attendees should not share the code or forward the email to others who did not actually attend the program. If they are not able to be verified on our webinar provider reports and registration list they will not receive credit. If you are joining only by phone, you will not be eligible for credit.
