HealthTrust presents…“Management of Inpatient Anticoagulation Therapy”

DOAC therapy has been in use for over a decade. However, there are still gaps in knowledge when it comes to proper management of therapy. Dosing in renal dysfunction, obesity, periprocedural management, as well as transitions between different therapies are all questions that clinicians must be able to answer. The presenter will review the management of inpatient anticoagulation therapy and characterize important pharmacokinetics and pharmacodynamics, including renal dose adjustments as well as dosing by level. The use of novel oral anticoagulation in patients at the extremes of body weight and current recommendations will be discussed as well as reversal in the case of emergent situations.

  1. Describe the coagulation cascade and identify where anticoagulation therapies act in the cascade.
  2. Differentiate the pharmacokinetics and pharmacodynamics between current anticoagulation therapies
  3. Identify parameters to monitor efficacy and toxicity of therapy and to appropriately dose anticoagulant therapy.
  4. List agents and indications for anticoagulation reversal.

  1. List reversal agents that require special handling
  2. Understand reversal agents and storage requirements
  3. Identify safety warnings for anticoagulation therapy and reversal
