HealthTrust presents…“Dosing Approaches to Optimize Cost-Effective Care of Patients”

Medication costs make up most of health-system pharmacy budgets and continue to grow faster than any other healthcare expenditure. This session will review cost-effective dosing approaches for three different medications: four-factor prothrombin complex concentrate, intravenous immune globulin and vancomycin. The strategy for four-factor prothrombin complex concentrate is to use a fixed-dose approach instead of dosing based on INR and weight. For intravenous immune globulin, the strategy is to use ideal body weight or even adjusted body weight to dose rather than actual body weight. Lastly, recently updated vancomycin guidelines recommend using AUC-monitoring rather than solely depending on trough levels.

  1. Identify a fixed-dose strategy for 4-factor prothrombin complex concentrate
  2. Calculate IVIG doses based on different dosing strategies
  3. Discuss appropriate therapeutic drug monitoring for vancomycin
