HealthTrust presents…“Pharmacists at the Pump: Anticoagulation & Bleeding Management in Mechanical Circulatory Support”

Anticoagulation is necessary in mechanical circulatory support (MCS) to prevent device thrombosis caused by artificial surface contact and the prothrombotic effects of the device. Despite MCS advancements, bleeding remains a common, yet severe complication, that increases hospital morbidity and mortality. Management in the intensive care unit requires a balance between device thrombosis with bleeding. MCS encompasses technologies such as Impella devices, intra-aortic balloon pumps (IABP), left ventricular assist devices (LVADs), and extracorporeal membrane oxygenation (ECMO). Due to the risk of clotting with MCS, anticoagulation is used to mitigate thrombosis; however, this poses the risk of bleeding as well. Bleeding remains a common complication across these devices. This presentation aims to summarize the management of bleeding complications, specifically focusing on anticoagulant use, reversal agents and other therapies for prevention of bleeding within this patient population.

Learning Objectives for Pharmacists & Nurses | At the end of this session, participants should be able to:

  1. Recall the incidence, pathophysiology and device-specific risk factors contributing to bleeding in patients with Mechanical Circulatory Support (MCS).
  2. Identify anticoagulant strategies used in patients receiving MCS to include thrombosis prevention, bleeding risks, and monitoring strategies.
  3. Recognize current and emerging evidence on reversal agents and novel pharmacologic therapies to treat and prevent bleeding in patients on MCS.

Learning Objectives for Pharmacists & Nurses | At the end of this session, participants should be able to:

  1. Recognize strategies for effective education for patients receiving anticoagulant medications in the treatment of Mechanical Circulatory Support (MCS).
  2. Identify patterns of nonadherence to anticoagulant medications.
  3. Recall anticoagulation medications and potential side effects for patients with MCS.
