Such misconnections happen when one type of medical device is mistakenly attached to another medical device that performs a different function. Inspired by continued reports of serious injuries and patient deaths associated with these misconnections\, the FDA recently issued a letter to enteral feeding tube manufacturers and distributors\, healthcare professionals and hospital purchasing departments recommending that hospitals and clinicians use enteral devices with connectors that meet the ISO (International Organization for Standardization) 80369-1 or ISO 80369-3 standard\, or that are otherwise designed to reduce the risk of misconnections. National survey results\, nursing literature\, and direct observation of nursing practice suggest significant variation in unit-based preparation of IV push medications and unsafe injection practices. While national emphasis has been placed on the improvement of IV infusion safety\, it is equally important to assist nurses in the identification and management of the risks associated with preparation\, management and administration of IV push medications. \nHear from nursing faculty as they describe these often-unrecognized risks\, relate national survey results associated with IV push medication use\, and discuss the current recommendations from the ISMP Safe Practice Guidelines for Adult IV Push Medications. Opportunities exist for hospitals to improve the cardiac arrest team’s performance and survival rate based on the 2018 American Heart Association (AHA) published “Get with the Guidelines” (GWTG) Cardiac Resuscitation Registry.
