SUMMARY:"Pharmacists at the Pump: Anticoagulation & Bleeding Management in Mechanical Circulatory Support"
DESCRIPTION:HealthTrust presents…“Pharmacists at the Pump: Anticoagulation & Bleeding Management in Mechanical Circulatory Support” \n \nDownload the content. \nPlease Note: Continuing education credit is only available to those attending the session live. CE credit is not available when accessing the program on demand. \nAnticoagulation is necessary in mechanical circulatory support (MCS) to prevent device thrombosis caused by artificial surface contact and the prothrombotic effects of the device. Despite MCS advancements\, bleeding remains a common\, yet severe complication\, that increases hospital morbidity and mortality. Management in the intensive care unit requires a balance between device thrombosis with bleeding. MCS encompasses technologies such as Impella devices\, intra-aortic balloon pumps (IABP)\, left ventricular assist devices (LVADs)\, and extracorporeal membrane oxygenation (ECMO). Due to the risk of clotting with MCS\, anticoagulation is used to mitigate thrombosis; however\, this poses the risk of bleeding as well. Bleeding remains a common complication across these devices. This presentation aims to summarize the management of bleeding complications\, specifically focusing on anticoagulant use\, reversal agents and other therapies for prevention of bleeding within this patient population. \nLearning Objectives for Pharmacists & Nurses | At the end of this session\, participants should be able to: \n\nRecall the incidence\, pathophysiology and device-specific risk factors contributing to bleeding in patients with Mechanical Circulatory Support (MCS).\nIdentify anticoagulant strategies used in patients receiving MCS to include thrombosis prevention\, bleeding risks\, and monitoring strategies.\nRecognize current and emerging evidence on reversal agents and novel pharmacologic therapies to treat and prevent bleeding in patients on MCS.\n\nLearning Objectives for Pharmacy Techs | At the end of this session\, participants should be able to: \n\nRecognize patients needing additional education from the pharmacist while receiving anticoagulant medications in the treatment of Mechanical Circulatory Support (MCS).\nIdentify anticoagulant medication schedules in the treatment of MCS.\nRecall anticoagulation medications and potential side effects for patients with MCS.\n\nPresenter: \n \nCourtney Cianci\, PharmD  | PGY-2 Critical Care Pharmacy Resident  |  Tristar Centennial Medical Center \nTarget Audience:  Pharmacists\, Pharmacy Techs & Nurses \nCE Credit: Approved for Pharmacists\, Pharmacy Techs & Nurses \nThis program is approved for CPEs by the Accreditation Council for Pharmacy Education (ACPE). SUMMARY:"Staph Meeting: A Case-Based Approach to Navigating Methicillin-Susceptible Staphylococcus Aureus (MSSA) Treatment"
DESCRIPTION:HealthTrust presents…“Staph Meeting: A Case-Based Approach to Navigating MSSA Treatment” \n \nDownload the content. \nPlease Note: Continuing education credit is only available to those attending the session live. CE credit is not available when accessing the program on demand. \nMethicillin-susceptible Staphylococcus aureus (MSSA) continues to be a leading cause of bacteremia and endocarditis within the United States and despite standard treatment approaches\, 90-day mortality is still markedly elevated. Current guidance promotes antistaphylococcal penicillins such as nafcillin and oxacillin as gold-standard for serious MSSA infections\, but emerging evidence now suggests cefazolin may offer comparable efficacy with a more favorable safety profile. The goal of this session is to evaluate the evidence comparing cefazolin to alternative MSSA treatments\, including antistaphylococcal penicillins\, other cephalosporins\, and vancomycin. Patient-specific considerations which guide us to use one agent over another will also be discussed. \nLearning Objectives for Pharmacists & Nurses  | At the end of this session\, participants should be able to: \n\nRecall the efficacy and safety of cefazolin versus anti-staphylococcal penicillins\, other cephalosporins and vancomycin.\nRecognize the clinical relevance of the inoculum effect when selecting betalactam therapy for Methicillin-Susceptible Staphylococcus Aureus (MSSA).\nIdentify evidenced-based guidelines for de-escalation from vancomycin or broadspectrum cephalosporins such as\, cefepime and ceftriaxone\, to cefazolin in\nconfirmed MSSA infections.\n\nLearning Objectives for Pharmacy Techs  |  At the end of this session\, participants should be able to: \n\nIdentify pros and cons with cefazolin when treating Methicillin-Susceptible Staphylococcus Aureus (MSSA) infections.\nRecall dosing frequencies between cefazolin and ceftriaxone.\nRecognize common preparation details with cefazolin.\n\nPresenters: \n \nGeorge Saied\, PharmD\, MBA  |  PGY1 Pharmacy Resident  |  TriStar Centennial Medical Center \n \nBreanna Wright\, PharmD\, MS  |  PGY1 Pharmacy Resident  |  TriStar Centennial Medical Center \nTarget Audience:  Pharmacists\, Pharmacy Techs & Nurses \nCE Credit: Approved for Pharmacists\, Pharmacy Techs & Nurses \nThis program is approved for CPEs by the Accreditation Council for Pharmacy Education (ACPE). SUMMARY:"The Great Load Debate: Transitioning from Parenteral Anticoagulation to Direct Oral Anticoagulants in the Treatment Phase of Deep Vein Thrombosis & Pulmonary Embolism"
DESCRIPTION:HealthTrust Presents…“The Great Load Debate: Transitioning from Parenteral Anticoagulation to Direct Oral Anticoagulants in the Treatment Phase of Deep Vein Thrombosis and Pulmonary Embolism” \n \nDownload the content. \nPlease Note: Continuing education credit is only available to those attending the session live. CE credit is not available when accessing the program on demand. \nThe risk of recurrent venous thromboembolism is greatest during the first week following an acute thrombotic event and remains elevated throughout the first 30 days. A meta-analysis of 15 studies confirmed that recurrence risk peaks in week one\, declines over weeks two to four\, and stabilizes after week five. Major direct oral anticoagulant (DOAC) trials addressed the early high-risk phase using intensified initial therapy. AMPLIFY and EINSTEIN used higher lead-in dosing\, whereas HOKUSAI-VTE and RE-COVER required 5-10 days of parenteral anticoagulation before transitioning to oral therapy. Notably\, most trial patients received less than or equal to 48 hours of parenteral anticoagulation\, which is shorter than typical real-world practice. This discrepancy creates clinical uncertainty regarding whether parenteral days of therapy should count toward the DOAC lead-in duration or whether the full oral lead-in regimen should still be\ngiven. Consequently\, practices vary: some clinicians adjust the lead-in period by subtracting parenteral days\, while others administer the full duration regardless of prior parenteral therapy. SUMMARY:"Beyond the Line: Oral Agents in the Treatment of Gram-Positive Blood Stream Infections"
DESCRIPTION:HealthTrust presents…“Beyond the Line: Oral Agents in the Treatment of Gram-Positive Blood Stream Infections” \n \nDownload the content. \nPlease Note: Continuing education credit is only available to those attending the session live. CE credit is not available when accessing the program on demand. \nCurrently\, gram-positive bacteremia is primarily managed with intravenous antibiotics for 14 days after the first negative blood culture. For some patients\, there are many barriers to using IV antibiotics for this duration. Emerging research is exploring the use of oral antibiotics\, but not as much for gram-positive bacteremia. The use of oral agents can provide the same level of care while avoiding common barriers to IV antibiotics such as availability\, transportation\, cost and side effects. This presentation will provide a brief overview of gram-positive bacteremia\, review the current standard of care with intravenous antibiotics\, and summarize key findings from recent literature evaluating oral agents. This information will help highlight the appropriate use of oral antibiotics for gram-positive bacteremia. \nLearning Objectives | At the end of this session\, participants should be able to: \n\nIdentify patient-specific factors for determining an appropriate candidate for oral antibiotics in the treatment of gram-positive bacteremia\nRecognize commonly used oral antibiotic options during the treatment of gram-positive infections along with strengths and/or limitations of their use\nRecall evidence-based treatment guidelines utilizing oral antibiotics for the patient with gram-positive bacteremia\n\nPresenter: \n \nMackenzie Hall\, PharmD  |  PGY1 Pharmacy Resident  |  Memorial Hospital of South Bend \nTarget Audience:  Pharmacists & Nurses \nCE Credit: Approved for Pharmacists & Nurses \nThis program is approved for CPEs by the Accreditation Council for Pharmacy Education (ACPE).
