Pharmacy personnel can play an important role in accurately documenting home medications\, reconciling medication orders\, providing additional recommendations on appropriateness of therapy\, and decreasing the burden on nursing staff. In 2023\, one 159-bed hospital in Tennessee initiated a successful pharmacy-driven medication reconciliation effort\, swiftly meeting enterprise standards and enhancing patient care with interdisciplinary collaboration.
