DESCRIPTION:HealthTrust presents…Virtual HTU: “Med ‘Wreck’ to Med Rec: Medication Reconciliation Quality Improvement & Program Development” \n \nDownload the content. \n*Note: Members who attended the live HTU event and applied for CE credit are not eligible to receive credit for the same program in a virtual format. In addition\, CE credit is not available when accessing the program on demand. \nEffective medication reconciliation is crucial for safe medication use. Pharmacy personnel can play an important role in accurately documenting home medications\, reconciling medication orders\, providing additional recommendations on appropriateness of therapy\, and decreasing the burden on nursing staff. In 2023\, one 159-bed hospital in Tennessee initiated a successful pharmacy-driven medication reconciliation effort\, swiftly meeting enterprise standards and enhancing patient care with interdisciplinary collaboration. Also in 2023\, other facilities in the Far West Division (FWD) with established Pharmacy-Led Medication Reconciliation programs created the FWD Med Rec Quality Improvement Workgroup to facilitate a collaborative approach to identifying and implementing quality improvement initiatives.
